Here is a sample letter that I received after writing to representative Kelly. The website that I used was http://www.house.gov/writerep/ and when you go there you can find different representatives that are in your state. You may get different information from different representatives. If you would like to view Representative Kelly's page then you can go to this site. It is http://www.house.gov/suekelly/.
