1. It really depends on what you mean by miniature black holes. How small is miniature? Stars of masses less than 1.4 of the sun will become white dwarfs when they die. However, since black holes can be created in theory, it is possible for a really small black hole to form. Their life span will not last very long since it will probably evaporate not long after it is created.
2. I believe that suppermassive black holes were formed at the early stages of the development of our universe or our galaxy. They are also the reason why galaxies exist (since a force has to keep all the stars rotating around it).
