Game is in the 7th inning. You want to bring in your reliever - the "set-up man", not the "closer" - you want him to pitch two innings for you, but he is scheduled to bat in the next inning; the 8, 9 and lead-off batting slots will come up. What do you do? Do you let the set-up man bat? DO you pinch-hit for him?
You make a double-sub/double switch. You bring in set-up man for the starting shortstop (bats 7th) and you replace your starting pitcher with your back-up shortstop. In the 8th inning, the back-up shortstop will bat in the pitcher's 9 hole, with luck, you will not have to bat or pinch hit for the pitcher, who is now in the 7th spot in the order and wouldn't come up 'til extra innings.
