The physicans' confidentiality will be kept under strict regulation. There is no question in which the physicians must include their name. When given the invitation to take this opinion poll, the physicians are instructed NOT to include their name, phone number, or address. If they would like to be informed of the results in a later date, they will should write their address on a small sheet of paper and ENCLOSE it with the letter. The mailing address will be seperated from the answer sheet; then randomized.

In the internet web site, the doctor is not given a chance to enter their name, phone number, or mailing address. If the participant should request the results of the project, they will join a seperate portion of the web site to enter their email address. When all responses are in, the email addresses will be randomized for future use.

Absolutely no names, phone numbers, mailing addresses, or email addresses will be distributed, sold, or open for public use. All private responses will be strictly confidential. It is imperative for the physicians to know that all of the questions are completely optional.
